I hereby apply for lay membership in the Conservative Congregational Christian Conference.

I wholeheartedly accept its Statement of Faith and Polity and agree to its Constitution and By-laws.

I will work with the other members of the CCCC for the advancement of Biblical Christianity throughout the world, according to Congregational principles.

I agree to meet with a representative of the CCCC, with the understanding that the representative will send a letter of recommendation to the Credentials Committee as a part of this application.
